There is a thread in Hanggliding.org (Hg Midair... Ugly). On the 5th page of the thread a well respected instructor did a great write-up mentioning his thoughts on the situation.

He also expresses his thoughts on what may have been the outcome if he were one of the pilots.

I had not heard of any deaths from these recalls, but that isn't the issue. Their actions are what stands out. If the quality of the carabiner was not an issue, then why send out recalls. Possibly an incomplete analysis to a seemingly significant event.

The pilot may have looked low, but maintained a steady course, while retaining plenty of energy.

Obviously the spot he landed is proof in the pudding.
