Think you should pay more attention to the details. The harness is CLEARLY not strapped down tightly. You can see right before they move to the door, the left strap has moved down off her shoulder and over her left arm.
As they are readying to depart the door, you can see there is way to much movement on the left portion of the harness. I suspect it should have been tightened a LOT more.
Suspect the the departure from the door was less than optimal seeing how she was clearly holding back and the tandem guy had to grab her arms so he could basically throw her out the door.
From the flapping of the skin that is exposed, I'd say that isn't a young person.
Good thing she kept her kness bent otherwise it looks like it would have had a much different outcome.
