Please tell what happened and how it might have been avoided. Names should be ommitted. This forum should help others learn from mistakes that caused or nearly caused a mishap.

Reporting Format

All reports should include the following information to be as thorough as possible.

Time & Date: Should help in determining the type of conditions one would normally expect during the same period.

Place: Preferably giving as much detail as possible if the site isn't well known to others.

Conditions: Wind speed and direction, temperature, cloud development, any other factors that may seem relevant.

Witnessed or saw outcome: Preferably we'd like to get first hand reports but sometimes second hand reports can be useful as well.
